* P411 membership ID with 2 recent Okays (within 90 days), please send an Appointment Request from your P411 account in addition to the booking form info.
If you do not have current references, please provide a photo with an unobstructed view of your drivers license. (REQUIRED)
If you don't have an employer, where did you last work? Are you Google-able?
Other items you can send if you're traveling: boarding pass, hotel info.
Once the screening process has been completed, I will be back in touch to confirm your appointment.
Please provide any handles you use on TER, P411, Twitter or other platforms. You may be asked to confirm ownership of your account, please send a private message from your account.
If you are new or don't have this information, I have other means of verifying you, so please submit as much of the above information as you have available or contact me for alternative methods. Thank you for understanding if additional screening information is requested. Safety, security and discretion (yours and mine) are paramount.
Repeat gentleman and those requesting longer appointments receive scheduling preference. For you "fly-by-the-seat-of-your-pants" kinda guys, get screened early, then when the whim hits, you're good to go.

Due to limited availability, touring appointments require a 50% deposit to hold your time. Pre-screening is also highly recommended. While I'll entertain same-day requests, it will be difficult to complete the screening process. If you want to ensure your time, send me your screening information early and reserve a time just for you.
I understand that things come up unexpectedly or that you may have to cancel or reschedule, but please understand that my time is a valuable commodity. If you cancel less than 24 hours prior to our appointment, a 50% CANCELLATION FEE will apply and same-day cancellations will incur a 100% CANCELLATION FEE. When on tour, once an appointment has been confirmed, a 100% CANCELLATION FEE will apply. Thank you for understanding. No show appointments will be assessed a 100% CANCELLATION FEE plus $100 (When on Tour, plus $200). Failure to comply with cancellation policies will result in immediate local, regional and national Blacklisting.
